Diabetic foot is defined as the condition in which the feet are involved in diabetes.The foot is involved in diabetes in two ways
-The nerves are affected
-The vessels are affected
In most of cases the both nerves and the vessels are affected
When vessels are involved
The vessels supplying the blood to feet are blocked in diabetes , resulting in poor blood supply and so wounds and pain.As all distal and smaller vessels are involved , no reconstruction is possible.We are the first microvascualr surgeons who showed that blood supply of the foot can be increased by using the alternate channels and first case we did was presented in the annual conference of microvacsualr surgeons in Mumbai 15 years back.We again presented a series of 165 cases in just concluded conference of microvascualr surgeons of India at Taj Deccan in hyderabad on July 2010 and told the delegates that such use of byepass channels can save many limbs from amputation
When nerves are involved
Involvement of nerves casuses sensory loss pain and deformities of the foot.Result is that patient can barely walk.Such deformities can be minimised if the nerve is released from compression behind the malleoli of the foot.The tretament was recommended by surgeons (Dellon) from USA and we followed him to do the largest series of nerve decompressions in India with results showing that future ulcerations can be prevented by nerve decompressions
What is our advice
We have noticed that nerve decompression not only releases the compression but increases the blood supply of the foot also.So we recommend all patients of diabetic foot to undergo nerve decompressions and along with that distal foot revscaularizaions ,After doning the same in 165 patients we lost only one foot and could save all of the feet.
Same is true to other conditions which cause decreased blood supply to the foot
